DUTIES:
The Home Care Therapist delivers premium, patient-focused treatment by overseeing the entire process of providing therapeutic care to a designated group of patients. This is done in collaboration with the physician and the patient care team. The Home Care Therapist evaluates patients and contributes to the development and implementation of personalized care plans. The main focus is achieving goals, engaging patients, and ensuring a positive patient experience. The Home Care Therapist is responsible for providing or coordinating most of the patient’s physical therapy to support patients throughout home health and different phases of their care.
Qualifications
REQUIREMENTS/EXPERIENCE:
Expertise in Home Health Therapy
- Proficient in computer skills and maintaining simple records in English.
- Utilizes various verbal and written communication methods to provide patient and family education that respects their cultural, ethnic, and personal needs or preferences.
- Collaborates with patients and families to teach appropriate self-care techniques.
- Submits documentation according to Agency policy.
- Knowledgeable about current federal, state, and agency regulations for patient care, including incident management and reporting.
- Maintains compliance with regulatory requirements and Legal Authority regulations.
- Participates in agency’s mandated activities, such as in-services, Quality Assurance and Performance Improvement (QAPI), and staff meetings.
EXPERIENCE:
- Preferably at least one year of experience in Home Health Care.
- Thorough understanding of general therapy theory and practice, with the ability to work autonomously.
REQUIREMENTS:
- 1 year of Home Health Experience
- Bachelor’s degree in Occupational Therapy from an accredited program
- Successful completion of the National Certification Examination for Registered Occupational Therapist
- State license in practicing state
- Reliable transportation and proof of valid automobile liability insurance
- Must possess a valid driver’s license
EDUCATION:
- Graduated from an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association.
- Preferably a member of the American Occupational Therapy Association.
