ALC Care Coordinator

Our organization is a well-respected non-profit entity that operates in multiple locations in the Saratoga Springs, NY area, providing senior citizens with various options for living arrangements, including Senior Living, Assisted Living, and Memory Care services.
We are currently in need of a highly skilled and experienced Registered Nurse (RN) Case Manager in Malta, NY to contribute to our ongoing success.

Job Summary:

In this role as a Case Manager, you will serve as a crucial component in delivering necessary services to our residents while adhering to the regulations set forth by the state of New York. Your primary responsibility will revolve around coordinating the efforts of our Personal Care Staff to ensure the smooth operation of medication systems, direct care services, and case management processes. Reporting directly to the Administrator, you will play a vital role in maintaining the well-being and independence of our residents while fostering a supportive and nurturing environment.

Location: Malta, NY / Ballston Spa, NY
Hours: Monday-Friday around 8a-5p

Duties and Responsibilities:

  • Monitor and schedule annual physicals for residents, ensuring compliance with applicable regulations.
  • Coordinate routine doctor appointments and facilitate care coordination among various healthcare providers.
  • Effectively communicate changes in medication and residents’ doctor visits to the Personal Care Supervisor using thorough case management notes.
  • Complete timely annual and 30-day case management assessments.
  • Provide personalized case management services to residents, promoting their independent functioning and personal choices, while conducting regular audits and supervision of case management processes.
  • Act as a liaison between residents, families, and different departments within the organization.
  • Make recommendations for additional services based on comprehensive social, physical, and mental evaluations of residents.
  • Maintain accurate and up-to-date resident information, including diet orders, resident rosters, and more.
  • Perform various office duties as required, including regular audits of Medication Administration Records (MARs) and Physician Orders.
  • Keep track of visits and paperwork for the podiatrist, ensuring that residents’ podiatric needs are met.
  • Manage resident charts, organizing current medication orders, diet orders, telephone orders, and the Hospital Emergency Book.
  • Assist with the admissions, discharges, and transfers of residents as necessary.
  • Conduct regular and timely care plan meetings.
  • Inform physicians about resident illnesses, obtain medication orders, and follow up with pharmacies and physicians regarding telephone orders on a daily basis.
  • Closely monitor residents to identify any sudden or progressive changes in behavior or appearance that may require medical attention or additional services.

Requirements:

  • Active Registered Nurse (RN) license
  • Familiarity with the New York State Regulations regarding case management and healthcare services.
  • Past experience in a supervisory or managerial role. This position serves as a Department Head.
  • Valid driver’s license with a clean record. Travel between locations may be required.
  • Strong organizational skills, attention to detail, and excellent communication abilities.
