Nurse Supervisor Home Care

Job Responsibilities:

• Conducts initial and continuous evaluation of patient and family to identify appropriate services
• Collaborates with patient, family, and care team for service coordination
• Supervises professional and support staff assigned to patient
• Maintains regular communication with patient and family for optimal outcomes

Requirements:

• Active RN license in New York State
• Bachelor's degree in Nursing
• Minimum of 3 years clinical experience
• Prior experience in home care
• At least 1 year of supervisory experience
• Strong observation, problem-solving, and communication skills
• Compassionate and ethical approach
